What Is OCD, and How Does It Work?

OCD, or obsessive compulsive disorder, has two parts. Obsessions are unwanted thoughts, images or urges that keep coming back and cause distress. Compulsions are the actions or mental acts done to relieve that distress, such as washing, checking, counting or repeating a phrase in your head. The National Institute of Mental Health reports that about 1% of US adults have OCD in a given year.

The pattern is a loop. The thought spikes anxiety, the ritual brings short relief, and the relief teaches the brain to repeat it. Over time the ritual takes longer and the thought gets louder.

OCD is not being neat. Most people with it know the fear is out of proportion and still cannot stop. It often starts in the teens or early adulthood, and many people live with it for years before anyone names it. See what OCD looks like beyond handwashing.

TYPES OF OCD

Types of OCD We Treat in Miami Lakes

OCD takes different forms, and the form affects what treatment targets. These are the types we evaluate and treat at our Miami Lakes office.

Contamination OCD

Fear of germs, illness or dirt, with excessive washing, cleaning and avoidance of places or objects. Rituals often grow over time.

Checking OCD

Repeated checking of locks, appliances or work to prevent a feared outcome, continuing well past what is reasonable.

Symmetry and Ordering OCD

A need for things to feel even or ‘just right’, with arranging, counting or repeating until the feeling passes.

Intrusive Thought OCD (Pure O)

Unwanted thoughts about harm, taboo topics or your character, with mostly mental compulsions such as reviewing or neutralizing.

Relationship OCD (ROCD)

Constant doubt about a partner or relationship that reassurance relieves only briefly, followed by more checking and questioning.

Religious and Moral OCD (Scrupulosity)

Fear of sinning or doing something morally wrong, with excessive praying, confessing or seeking reassurance from others.

Signs and Symptoms of OCD in Adults

OCD becomes a clinical problem when obsessions and compulsions take more than an hour a day or get in the way of work, relationships or daily life. Several of these together is a reason to book an evaluation.

How We Diagnose OCD in Miami Lakes

OCD is diagnosed through a clinical interview. There is no blood test or scan for it. The provider asks what thoughts and urges come up, what you do in response, how much time it takes each day and what you avoid.

Mental compulsions get asked about directly, since nobody can see them. Many people say they have no rituals because they do not wash or check, then describe hours of mental reviewing. A rating scale such as the Y-BOCS is often used to score severity.

Anxiety, depression, ADHD and tic disorders are screened for, because they commonly overlap with OCD. You do not have to describe your intrusive thoughts in detail. The type and pattern are enough.

The visit is the same in person or by video. See our psychiatric evaluation page for how evaluations work.

What Causes OCD and Who Is at Risk?

No single cause has been found. OCD is linked to a mix of genes, brain circuits and life events.

Family history. A close relative with OCD raises your risk. Brain imaging studies point to differences in circuits that handle worry, habits and error detection, though that research is still developing.

Stress and life changes. A new baby, a move, loss or illness can trigger symptoms or make them worse. Some people have no clear trigger at all.

OCD usually begins in the teen years or early adulthood and is often hidden, so many adults first seek help in their 20s or 30s. It overlaps with ADHD, anxiety and depression. Read when ADHD and OCD overlap and why mental health conditions get confused.

TREATMENT OPTIONS

OCD Treatment Options at Mental Health First

Treatment depends on the type of OCD, how severe it is, your health history and what you prefer.

Medication Management

SSRIs are the usual first medication for OCD. They are often used at higher doses than for depression and can take 8 to 12 weeks to show full effect. See our medication management page.

Ongoing Psychiatric Care

OCD treatment needs time and regular check ins. Visits track symptoms, side effects and dose changes, and are closer together at the start and further apart as symptoms improve.

In Person and Telepsychiatry Appointments

Visit our Miami Lakes office or use telepsychiatry by secure video anywhere in Florida. Same week appointments are available, so you do not wait weeks to start.

How OCD Is Treated: Medication, ERP and What to Expect

Two treatments have the strongest evidence for OCD: exposure and response prevention (ERP), a type of talk therapy, and SSRI medication. Many people use both. Mild OCD may respond to ERP alone. Moderate or severe OCD, or OCD that makes therapy hard to start, often begins with medication.

ERP is delivered by a therapist trained in it. You face the feared thought or situation in steps and practice not doing the ritual. Mental Health First provides evaluation and medication management. ERP is provided by a separate therapist, and the two can run side by side.

OCD often needs higher SSRI doses than depression, and full effect can take 8 to 12 weeks. If the first medication does not help after a full trial, the dose, the medication or an added medication can change.

A usual first three months: evaluation in week one, a check at four to six weeks for side effects and early response, and a review at eight to twelve weeks to decide whether to continue or change. Read how OCD is treated for the full guide.

    HELPFUL ANSWERS

Questions About OCD Treatment in Miami Lakes

Both are possible, and they overlap. General anxiety is loose worry about many things. OCD is narrower and ritualized: the checking or counting follows a pattern, and it brings a short window of relief before the urge returns. If checking the stove twice has turned into eight times before you can leave the house, bring it to an evaluation. See our anxiety treatment page.

No. The type of thought and the pattern are enough. The provider does not need graphic detail to make a diagnosis.

Often the same class, SSRIs. OCD usually needs higher doses and more time. See our medication management page.

No. Many people live with OCD for years before a diagnosis. Treatment can reduce symptoms at any age.

They count. Replaying conversations, mental reviewing, silently repeating phrases and checking your feelings are all compulsions. The provider asks about them directly. Describe them as best you can.

Early changes can show in the first weeks, but full effect often takes 8 to 12 weeks, and sometimes longer with dose changes.

Not necessarily. It depends on how long you have had OCD, how severe it is and how you respond. Whether to continue or lower medication is reviewed with your provider over time. Do not stop on your own.

Coverage varies by plan. Our team checks your benefits before your first visit. See our insurance page or call 954-620-8808.

Two treatments have the strongest evidence: exposure and response prevention (ERP), a type of therapy, and SSRI medication. Many people use both. Which to start with depends on severity and what is available. Read how OCD is treated.

It runs in families. A parent or sibling with OCD raises your risk, but most people with a family history do not develop it, and many people with OCD have no family history.

Both involve fear. Generalized anxiety is broad worry about real life concerns. OCD is built on intrusive thoughts and rituals done to cancel them. They often occur together. See our anxiety treatment page.

They can look alike, since both cause trouble finishing tasks, but the causes differ. ADHD is trouble regulating attention. OCD is a fear based loop. They can also occur together. Read when ADHD and OCD overlap and see our ADHD treatment page.

Hoarding disorder is now diagnosed separately from OCD, though the two can overlap. It is evaluated on its own.

It rarely goes away without treatment, and symptoms tend to shift and return under stress. Treatment lowers symptoms for most people, though it often continues as long term management.
